heat transfer (convection and conduction) on an internal boundary

Hello,
I use the earth science module to simulate heat transfer in a fractured pourous media. In the pourous media subdomain there are darcy velocities u, v, w and on the internal boundary fracture velocities ub, vb and wb.

Is there anybody who have some experiences to formulate the convection and conduction equation as weak and dweak expressions (weak PDE) on the boundary?
