Nanofiltration boundary conditions

Hello

I want to model nanofiltration. I have difficulty for defining boundary condition at the
membrane-permeate interface. At the membrane-permeate interface (delta x) in the membrane side, concentration is not known for the membrane subdomain. For defining this boundary and also for solving the main equation, concentration of permeate side is needed. I think I should define a subdomain for Cp (permeate concentration) as a point subdomain. The subdomain and boundary conditions are illustrated in the attached file. the problem is defining boundary condition at the membrane-permeate side as a function of Cp.
