Coupling 1D Pipeflow to 2D axisymmetric Porous Media

I have used this average and integration operator coupling approach with 1D pipeflow to 3D k-epsilon coupling before with success.
When the model is run, the solution oscillates continuously but never converges. Plotting results while solving shows that there is an issue at the porous media inlet. The porous media inlet condition is based on integrating the pipeflow velocity at that point to the 2D inlet. Flow is not entering and passing through the porous media section like it should, and I'm not sure why that is. My model is attached. Any suggestions on how to fix this boundary inlet condition would be appreciated.
