Coupling problem

I am trying to solve a problem in which heat is transferred from one boundary ( which is say at 100 deg C) to other boundary of a solid model, there by to a cylindrical surface through its edge ( the cylindrical surface is in contact with solid model) . I have set heat trsfer in solids physics for the solid model and heat trsfer in in thin shells physics for the cylindrical surface. Now how can i tell the system that heat has to get conducted in cylindrical surface as well. Is there any way give the output temp in solid boundary as input to the cylindrical surface edge.
